MEDICAL CONSULTATION–
GENERAL GUIDELINES, RULES, FEES, TIMINGS
TO BE READ BY ALL PATIENTS PRIOR TO SEEKING CONSULTATION
I) CONSULTATION CHARGES:
Patients will be required to pay the fees according to the following table wef 01 Jan 2014 prior to the clinical examination and consultation:-

NOTE:
(i) Dr Arvind Chopra (AC) will provide consultations to a limited number of patients daily depending upon his availability and time schedule.

(ii)All patients will be first screened by doctors and paramedics prior to specialist consultation

(iii) Due to time constraints, if you accept consultation from another specialist/doctor, you will be charged less and depending upon your clinical condition, Dr Chopra may still review your prescription with the examining doctor.

(iv) Appointment will be provided 10-15 minutes consultation time

(v) Patients seeking consultation outside the outpatient timing hours of the center (see below) will be accepted as per the discretion of the specialist and manager and will be charged extra
( Rs 250-500/=).

(vi) Home visit may be agreed upon as per the specialist/doctor discretion and will be charged at five times the relevant fees described above under ‘fees without appointment’.

(vii) In case of a special injection or procedure provided by a specialist, extra charge will be added to the consultation charge excluding center charges and material charges.

(viii) Receipt will be issued for every payment and its patient’s responsibility to collect the same at the time of payment. It is not obligatory to issue duplicate bills and duplicate receipts at a later date.

(ix) Work Timing: 8am to 630 pm daily on Monday to Friday, & 8am to 4 pm on Saturday (lunch break is from 1.30 -230 pm).

II) GUIDELINES & RULES:
(1) Patient records and medical details: Patients are advised to cooperate fully with the staff in providing all necessary medical and other relevant information that may be required to complete the case record forms. Patients must provide details of any kind of allergy to drugs experienced in the past. Patient may be refused medical examination if unwilling to provide the necessary information to complete the case record form prior to consultation. Patients must bring reasonable number of old medical records, in particular the previous visit record, at every consultation visit.

(2) Appointment will be given to eligible patients as per the center schedule and will be for duration of 10-15 min. 10-12 appointment slots are available daily from Mon to Fri , distributed between morning and evening hours. Appointments are given on first come first serve basis. Special appointments may be given during the morning hours on Sat as per the discretion of the manager and slots for the same will be released on Thursday and Friday.
Appointment must be reconfirmed at least two days in advance from the reception (Tele 26344099). Patients seeking appointment must provide full contact details including telephones and postal address to the receptionist in the slip provided for the purpose. Patient must report 30 mins prior to the appointment time for the records to be completed/ updated. The waiting time for an appointment patient in certain circumstances may be a maximum of 45 minutes. Consultants/ Management reserves the right to cancel any appointment or close the clinic without assigning any reason or explanation whatsoever but every attempt will be made to inform the patient concerned well in advance. Patients are advised to telephone the clinic on the day of the appointment prior to reporting for the same.

(3) Absence of Rheumatologist (AC, VLJ, VK): Patient will be charged a uniform rate of Rs 300 and Rs 200 respectively for an initial visit and a follow up visit if examined and advised by assistant rheumatologist in the absence of consultant rheumatologist. Dr. Arvind Chopra (AC) & Dr.V.Lagoo Joshi (VLJ) are specialist Rheumatologist. Dr V Kunjir (VK) is also specially trained to examine children with arthritis and rheumatism ( pediatric rheumatology) and may charge consultation fees as per the above schedule.

(4) Special needs and concessions: Though patients without appointment will be generally seen on first come first serve basis, a patient with special need may be examined out of turn as per the discretion of the doctor. Patient or his/her next of kin or accompanying person is responsible for informing the reception while registering for consultation any kind of personal need (e.g. wheel chair) or care that is required which will be provided depending upon the availability of such a facility. Doctors (medical, dental and veterinary) and their immediate family and parents, nurses, medical and nursing students, will be provided free consultation outside the appointment timings and 50% concession on the appointment charges. All serving and retired personnel from military, paramilitary and police services will be given 20% concession. Any other personnel may be considered for a concession at the discretion of the deputy director (Ms Anuradha), coordinator (Ms Manjit) and patient relation officer & Manager (Ms Rosemary), and the undersigned

(5) CRD AND HDH are private specialized medical outpatient facilities and are not associated with any other hospital or other similar medical association or institution. The timing for outpatient services will be 8am to 630 pm daily on Monday to Friday, & 8am to 4 pm on Saturday (lunch break is from 1.30 -230 pm). This center is not recognized by CGHS, ESI, or any other State or Central Government medical scheme. This center does not cater for any inpatient or emergency medical care, and does not have any surgical facility. Only medical certificates of routine patient care can be provided.

(6) Right of Admission and Consultation: The management and its senior medical staff and executives reserve the right to deny entry or admission or consultation to any person coming to Hermes Doctor House (HDH) and Center for Rheumatic Diseases (CRD) without assigning any reason. This right of refusal also includes providing medical certificate to any patient. Patients may be refused entry or consultation depending upon the availability of doctors, paramedic assistants and other relevant facilities on any particular day. Generally, the number of ‘new’ patients will be restricted to 15 and that of ‘old’/follow up patients to 40 during the regular working days (Mon to Fri). Also, no fresh cases will be accepted after 11am unless a special situation is duly authorized by a specialist. For all practical purpose, the receptionist is authorized to limit the number of patients for consultation on any particular day as per this guideline.

(7) The management is not responsible for any kind of material loss or personal injury that the patient may suffer in the center premises (HDH, CRD) during his/her consultation visit.

(8) The decision of the Director (Dr Arvind Chopra) will be final and binding in case of any kind of dispute arising out of the matter contained in this circular. In the absence of the Director, the final decision on the latter issue will be taken by Deputy Director (Ms Anuradha V) or in her absence by the Coordinator (Ms Manjit S)/ the senior most doctor available for the day.
